PeakHealth

Build strength, confidence, and independence

PeakHealth is HomeTeam’s healthy aging program. It is designed to help older adults improve their strength, balance, mobility, fitness, and confidence with physical activity.

The program combines guided activity, practical education, and support from the HomeTeam clinical team.

PeakHealth is not about being “fit enough.” It is about building the capacity and confidence to keep doing the things that matter to you.

PeakHealth Cohort 1

What is PeakHealth?

PeakHealth is an eight-week, group-based program for eligible HomeTeam patients.

Participants work on areas that are important for maintaining health and independence as we age, including:

  • Strength
  • Balance
  • Mobility
  • Cardiovascular fitness
  • Confidence with movement

Activities can be adapted to different abilities and starting points. The focus is on progress, not performance.

Who is PeakHealth for?

PeakHealth is intended for HomeTeam patients who may benefit from improving their physical strength, mobility, balance, fitness, or confidence with activity.

You do not need to exercise regularly or consider yourself particularly fit. Some participants are already active; others may have become less active because of illness, a fall, loss of confidence, or changes in their health.

Our clinicians recommend PeakHealth when they believe it may be a good fit for a patient’s health, goals, and abilities.

PeakHealth is currently available to HomeTeam annual-fee patients.

PeakHealth Intro

Why does it matter?

Strength, balance, mobility, and fitness all contribute to our ability to move safely, complete everyday activities, participate in our communities, and remain independent.

PeakHealth is part of HomeTeam’s broader focus on maintaining function: helping patients continue doing the things that are important in their day-to-day lives.

What should I expect?

PeakHealth is designed to be practical, supportive, and achievable.

During the program, participants work at a level appropriate for them, with a focus on building:

  • Strength for everyday activities such as standing, walking, climbing stairs, and carrying objects.
  • Balance and mobility to support safe, confident movement.
  • Fitness to improve capacity for sustained activity.
  • Confidence to help make physical activity a realistic part of everyday life.
